The Environmental Protection Agency of Liberia (EPA) informs the general public that it is actively investigating a tragic explosion that occurred at approximately 6:38 PM on February 17, 2025 at the Oxygen Plant of Sethi Ferro Fabrik Steel Factory, located in the Monrovia Industrial Park, Old LPRC Road, Gardnesville.
Following reports of the incident, the EPA promptly dispatched a high-level delegation to conduct a preliminary assessment of the accident. Initial findings indicate that the explosion originated from an Oxygen Cylinder that was being refilled. A worker, identified as Saah J. Samukai, was near the affected cylinder at the time of the explosion and was found lying motionless on top of the cylinders. It was confirmed that he tragically lost his life due to the incident.
The EPA extends its deepest condolences to the family, friends, and colleagues of the deceased during this difficult time. The extent of damage to the oxygen plant and surrounding equipment is currently under review, and a full-scale investigation is underway to determine the cause(s) (including if there were negligence on the part of the company) of the explosion. The EPA remains committed to ensuring public safety and regulatory compliance in all industrial operations.
The Agency assures the public that a comprehensive report will be released once the full investigation is completed. Further updates will be provided as more information becomes available.
